- ADMISSION AND PROMOTION/RETENTION FOR:
Bachelor of Science in Nursing
- ADMISSION
A. Regular Enrollees Requirements:
- Qualify in the UNP College Admission Test, with a rating of 80% or higher.
- Submit the following documents:
a. Photocopy of H.S. Diploma
b. Photocopy of Form 138
c. Photocopy of Birth Certificate
d. UNP CAT result
e. Certificate of Good Moral Character from High School Principal.
f. Marriage Certificate, if married
g. Certificate of Physical Fitness from University physician.
- Pass the interview.
B. Transferees/Shifters Requirements:
- Qualify in the UNP College Admission Test.
- Non-disqualification from other courses/college.
- Submit:
a. Certificate of honorable dismissal
b. Photocopy of Birth Certificate
c. Certificate of Good Moral Character from last school attended.
d. Marriage Certificate, if married.
e. Certificate of Physical Fitness from university physician.
PROMOTION / RETENTION
A. No student with any deficiency at any particular level shall be issued the certificate or diploma. If a student fails in a subject which is a prerequisite to another subject, then he/ she can not enroll in that subject. Example English 1 to English 2.
B. Any student who fails twice in any major subject will not be allowed to continue. A student shall be allowed to repeat a failed subject only once.
C. A student who fails in any major or preparatory subjects will not be allowed to enroll in the more advanced subjects. He can only enroll in the minor subjects , and the subject (s) failed..
D. Failure in 3 or more subjects in a semester will be a ground for dismissal from the college. If a student accumulates nine (9) failures at the end of his 2nd year, he can no more enroll in the university.
E. A legally married student who gets pregnant during the semester shall go on leave to protect her health and so as not to jeopardize her school work. However, should she decide to continue the course, she must sign a waiver sparing the university of any liability. However, she can only enroll in the general education/minor subjects. After giving birth, a clearance from the attending Obstetrician that she is physically fit to continue both in the clinical and classroom respectively shall be submitted.
F. The student is required to accomplish twenty (20) actual delivery cases in the midwifery program in preparation for the Midwifery Board Examinations.
G. The student must undergo the COPAR training prior to clinical work.
H. The student must undergo general orientation in the hospital before the clinical experience.
ABSENCES
1. A student can only incur absences equivalent to 20% of the total number of hours required for a given subject.
2. A student must secure a Pass Slip from the guidance counselor for any incurred absence before he/she can be admitted to his/her class.
3. If the absence is due to illness, the student has to secure also a medical certificate or a letter from the parent or guardian which will be presented to the guidance counselor.
4. For Student’s absences during clinical duty, the following policies apply:
a. Excused Absences
A student is required to have a make-up duty, where 1 day of absence is equivalent to 1 day make-up duty. b. Unexcused Absences
The student is required to have a make-duty, where 1 day of absence is equivalent to 3 days make-up duty.
A student who is excused from his hospital/community duty for a college/university activity but fails to attend must go for a make-up duty and for a fee. The ratio is 1:3. The make-up fee is based on the current rate of the clinical instructor concerned, to be paid to & receipted by the level coordinator.
5. For tardiness, the following applies:
• Three consecutive records of tardiness will be equivalent to a half-day duty considered unexcused.
ACTS OF NEGLIGENCE/MALPRACTICE
Any student who commits an act of negligence or malpractice in the Clinical Work Experience shall submit an Incident Report duly signed by the student, Clinical Instructor and the staff nurse on duty to be submitted to the Dean of the College. A Committee formed by the Dean shall investigate and evaluate the incident and apply appropriate sanction.
REVIEW CLASS
1. A preliminary review class for the Midwifery and Nursing Licensure Examinations will be conducted in the university by the college during the semestral break and by a Review Center during weekends for the second semester.
2. The formal review class will be in the university on weekends for the 2nd semester and at the Review Center one (1) month before the Licensure Exams.
3. A review fee will be collected from the students to be paid directly to the Review Center.
